Implications for practice and research
Nurse-led transition interventions can help young people with special healthcare needs be more educated and prepared for the self-management of their condition as adults.
A programme of research is needed to demonstrate the effects of such interventions on later health outcomes.

As children with special healthcare needs reach adulthood, they must transition from paediatric to adult care.
