A discharge planning intervention improved outcomes in older people admitted to hospital for hip fracture after a fall

METHODS
Design:
randomised controlled trial.
Allocation:
unclear allocation concealment.
Blinding:
blinded (outcome assessor).
Follow up period:
3 months after discharge.
Setting:
a medical centre in Taiwan, China.
Patients:
141 patients ⩾65 years of age (mean age 77 y, 69% women) admitted with hip fracture because of a fall. Patients who were seriously ill or cognitively impaired were excluded.
Intervention:
70 patients were allocated to a 3 month individualised intensive discharge planning intervention that was delivered by a nurse and included hospital visits, 1 home visit, telephone availability, and weekly telephone contact to address concerns of patients and caregivers and monitor progress; ongoing assessment of need …
