Enalapril and captopril increased mortality compared with ramipril in elderly patients with heart failure

QUESTION

Does type of angiotensin-converting enzyme (ACE) inhibitor prescribed affect mortality in elderly patients with heart failure?

METHODS

Design:

retrospective cohort study using government administrative databases, with mean follow-up of 2.1 years.

Setting:

Ontario, Quebec, and British Columbia, Canada.

Patients:

43 316 patients 65–105 years of age (median age 78 y, 52% women) who were discharged from hospital over a 4.25–year period with a primary diagnosis of heart failure, had not been admitted to hospital for this indication in the previous 3 years, and filled a prescription for an ACE inhibitor within 30 days of discharge.
